Incident reports often start with the person or thing affected, not the unknown or unimportant actor. The past simple passive keeps completed facts and evidence clear while allowing responsibility to be included when it matters.

Choose the passive when the incident result matters more than who performed the action.

Begin with information focus. The passive is useful when the affected subject or result should come first; it is not automatically more formal or better than the active.
Use was or were + past participle and keep time or evidence details outside the verb phrase.

Keep auxiliary agreement and participle accuracy separate. Prompt with “one or more?” for was/were and “third form?” for irregular participles before giving a model.
Include by + agent only when it adds responsibility, evidence, source or an important contrast.

If removing the agent changes responsibility, evidence or a needed contrast, keep it. If the reader loses nothing important, omit it.
Do not require by-phrases merely to show grammar. Evaluate whether the agent helps the listener understand accountability or trust the incident evidence.

Keep the recommendation distinct from completed facts. Require evidence for the incident report and a proportionate, clearly justified response.
Give a factual incident update, answer one evidence question and improve the highest-risk fact.
