Your Goal

By the end of this lesson, you can report six completed incident facts with accurate passive forms, decide whether to include the agent, and recommend one response.

Why this lesson matters

Incident reports often start with the person or thing affected, not the unknown or unimportant actor. The past simple passive keeps completed facts and evidence clear while allowing responsibility to be included when it matters.

PROGRESS FOCUS What improves today?
MEAN · MeaningLead with the affected subject and keep fact, evidence and responsibility distinct.
FORM · FormControl was/were + past participle and include by + agent only when useful.
USE · Real useDeliver a six-fact incident report, answer a verification question and recommend a response.

Your 30-minute route

4 · 5 minListen and reconstruct
5 · 5 minPlan six facts
6 · 6 minReport and retry
Teacher Lesson Plan

Teach the passive as an information-focus choice. Check affected subject, completed event, evidence and agent value before scoring form.

StageTeacher moveEvidence
1 · FocusChoose active or affected-subject focus.Eight decisions
2 · FormBuild was/were + participle.Ten forms
3 · AgentKeep only useful agents.Eight decisions
4 · ListenRecover event and evidence.Two maps
5 · PlanSequence six verified facts.Incident map
6 · PerformScore MEAN/FORM/USE and retry.Report + retry

Stage 1 · Focus on the affected person or thing

Choose the passive when the incident result matters more than who performed the action.

4 minutes
STUDENT TASK
Active factPassive incident factMessage focus
A driver damaged the case. The case was damaged.affected equipment
Staff closed two entrances. Two entrances were closed.changed access
The team delayed the opening. The opening was delayed.event result
A technician found the fault. The fault was found by a technician.finding + useful source
  1. Read eight incident facts and underline the affected person or thing.
  2. Decide whether active or passive gives the listener the more useful starting point.
  3. Transform six facts whose result should lead the message.
  4. Keep two active when the responsible actor is the main news.
Finish evidenceEight focus decisions and six accurate passive incident facts.
Teacher Help · Stage 1

Begin with information focus. The passive is useful when the affected subject or result should come first; it is not automatically more formal or better than the active.

Stage 2 · Build accurate completed-event forms

Use was or were + past participle and keep time or evidence details outside the verb phrase.

5 minutes
STUDENT TASK
SubjectPast passive frameExample
singular / uncountablewas + past participleThe equipment was checked at 9:15.
pluralwere + past participleThree boxes were moved.
negativewasn’t / weren’t + past participleThe warning wasn’t recorded.
questionWas/Were + subject + past participle?Were the doors locked?
  1. Choose was or were for ten incident subjects.
  2. Supply the correct regular or irregular past participle.
  3. Add a precise time, location or evidence detail to six facts.
  4. Repair four forms that use the past tense instead of the past participle.
  5. Ask and answer two passive verification questions.
Finish evidenceTen completed forms, six evidence details and four repairs.
Teacher Help · Stage 2

Keep auxiliary agreement and participle accuracy separate. Prompt with “one or more?” for was/were and “third form?” for irregular participles before giving a model.

Stage 3 · Decide whether the agent is useful

Include by + agent only when it adds responsibility, evidence, source or an important contrast.

5 minutes
STUDENT TASK
Agent statusDecisionExample
unknownomitA window was broken overnight.
obvious or unimportantomitThe fire was controlled quickly.
responsibility mattersincludeThe area was closed by the venue manager.
evidence/source mattersincludeThe fault was confirmed by an engineer.
  1. Audit eight passive incident facts.
  2. Remove agents that are unknown, obvious or irrelevant.
  3. Keep four agents that establish responsibility, evidence or source.
  4. Explain the information value of every kept agent.
  5. Change one sentence back to active because the actor is the main focus.
Check the agent decision

If removing the agent changes responsibility, evidence or a needed contrast, keep it. If the reader loses nothing important, omit it.

Finish evidenceEight agent decisions, four useful agents and one active rewrite.
Teacher Help · Stage 3

Do not require by-phrases merely to show grammar. Evaluate whether the agent helps the listener understand accountability or trust the incident evidence.

Stage 4 · Listen and reconstruct two incidents

Recover affected subject, completed event, evidence, useful agent and response.

5 minutes
STUDENT TASK
American English · Joanna

Delivery incident

Identify each affected subject, passive event, useful agent and response.

British English · Brian

Event incident

Recover the incident facts and decide which agent information is useful.

PassRecordQuestion
1 · Resultaffected person/thing + eventWhat happened?
2 · Evidencetime, place or sourceHow do we know?
3 · Responseaction already taken / next actionWhat should happen now?
  1. Listen once and map the result of each incident.
  2. Listen again and recover six completed passive facts.
  3. Mark which agents are included and explain why.
  4. Retell each incident with one clear response recommendation.
Finish evidenceTwo incident maps, six reconstructed passive facts and two response options.
Teacher Help · Stage 4

The reviewed neural voices provide accent variety. Score the affected subject and event meaning first, then auxiliary, participle and agent choice.

Stage 5 · Plan a six-fact incident report

Sequence verified facts before recommending one proportionate response.

  2. Plan six completed facts with accurate time or evidence details.
  3. Include only agents that add responsibility or evidence.
  4. Order the report from context to impact, evidence and action taken.
  5. Prepare one response recommendation and one verification question.
Finish evidenceA six-fact incident map with agent decisions and one recommendation.
Teacher Help · Stage 5

Keep the recommendation distinct from completed facts. Require evidence for the incident report and a proportionate, clearly justified response.

Stage 6 · Final Practice: report, verify and recommend

Give a factual incident update, answer one evidence question and improve the highest-risk fact.

6 minutes
STUDENT TASK
FINAL PRACTICE

Set context → report six facts → show evidence → recommend → clarify → retry

  1. Deliver a 75–90 second incident report with six completed passive facts.
  2. Use accurate was/were + past participle forms and clear time or evidence details.
  3. Include an agent only when responsibility, source or evidence makes it useful.
  4. Recommend one response and give one reason.
  5. Your partner asks one verification question about evidence, time or responsibility.
  6. Receive one MEAN, FORM or USE target and retry only the highest-risk fact or recommendation.
Incident model: “At 8:40, two equipment cases were delivered to the west entrance. One case was damaged, and the problem was recorded by the event coordinator. The case was moved to a secure area, the supplier was contacted and the opening was delayed by fifteen minutes. We recommend a delivery check at the loading point because damage can then be confirmed immediately.”

Score the first independent performance

MEAN · MeaningThe listener can identify six completed incident facts, supporting evidence and the recommended response.
FORM · FormWas/were, past participles, subject agreement and optional by-agents remain accurate enough.
USE · Real useThe learner reports the incident, answers a verification question and recommends one proportionate response.
0 · Not yetKey facts, time or responsibility remain unclear.
1 · With supportThe report works after one subject, participle, evidence or agent prompt.
2 · IndependentSix facts, agent decisions, verification answer and recommendation work without a model.

Result: 0–2 Rebuild · 3–4 Developing · 5–6 B1 performance evidence.

Finish evidenceSix completed incident facts + useful agent decisions + evidence answer + one response recommendation + focused retry.
Teacher Help · Stage 6

Score the first independent report. Give one target only, then record whether the retry improves incident meaning, passive form or evidence-based reporting.
